Healthcare Passport

Following Learning Disabilities Week 2025, we are pleased to have made new contacts at the Sutton LD Conference on Tuesday June 17th at the Phoenix centre.


We met the new Sutton SEND Forum, run by volunteer parents from Sutton to represent parent Carers of children with SEND. At this time there is no website, but they are available on Facebook or email.


We also wanted to share the Healthcare Passport template which is useful for completion by someone with a learning disability and/or their family or carer,  prior to a hospital admission for example, to share important information about care, needs and preferences.

The Healthcare Passport can be downloaded from ... and personalised before printing, or printed and completed by pen.

Based on a previous ‘Healthcare Passport’ the version below is designed to be used for people within a variety of settings. This content was revised and updated by Learning Disability Service at Epsom & St Helier University Hospitals NHS Trust, Sutton Learning Disability Health Facilitator and Service Improvement Programme Manager and the Clinical Health Team for Adults with Learning Disabilities, Sutton Health and Care.


In the near future, they will be exploring whether the healthcare passport can be made further accessible beyond online/word documents and printable versions, e.g. QR code scanning in a variety of healthcare settings.  Watch this space!
